Does your display tank have live rock and inverts, as that would make medicating a bit tricky. I'm curious, was the royal gramma's belly like that when you first brought it home, or has this developed since? Given that it's eating so well, I'd be inclined to hold off on treatment for now. You're gonna wanna keep it chill, no sudden moves. Use that bucket with some tank water, not fresh. Toss in the air pump, maybe wrap the bucket in a towel to keep temp stable. Bristlenose are tough, but they don't love changes. How big is the pleco? If it's over 3 inches, you might wanna skip ...

I'm considering adding a cleaner shrimp to the tank, given how often my royal gramma ends up with debris stuck to it from hiding in the rockwork. If there are indeed eggs nestled within that latest mat of Chaeto, the urchin or tiger cowrie will likely put an end to this behavior soon enough, once they find their way over to it. Thanks for the insight, zenoxa. That behavior's definitely been explained now. It's created several neat little mats of Chaeto, but unfortunately my urchin's had a go at a couple of the older ones. Now I just have to hope that my gramma regrows those lost scales.

Melafix isn't gonna cut it for ich, it's a mild antibiotic that only targets certain bacteria and fungi - ich is a protozoan, so it's basically useless in this case.
