My most terrifying experience

I just got home and found one of my firtideflow in the filter corner of my 50 gallon tank - I freaked out. Luckily my brother was able to rescue him. Has anyone else ever had a fish somehow get into their filter area?
Also, I've got a blue tang in the tank with my 2 occelaris clowns, 4 firtideflow, 1 pistol shrimp, 1 wheelers watchman goby, and a new bicolor blenny I just added on Saturday along with a brain coral and a torch coral. I noticed today that my blue tang is missing a bit of his back fin - not super noticeable but it's definitely a couple of slivers off. Before adding the new fish, he was fine. I'm wondering if maybe the filter caused it or possibly the blenny, but the blenny doesn't seem too interested in the other fish, he's more into eating all the algae on the back glass that I couldn't reach.
