Malawi deaths: seeking help to stop the loss

I desperately need help. My Jacob is on the verge of death and I'm at a loss for what to do. I've been trying to care for him but things just seem to be getting worse. I've read through some of the previous posts on this topic but I'm still unsure of the best course of action. I'd really appreciate any advice from more experienced aquarists.
